New Client Information

My office is located at:

 801 Samish Way, Bellingham WA, 98225.

Once you enter the main doors, please have a seat in the main lobby waiting room. For your first session with me, I will come greet you in the waiting room and take you back to my office. I will also show you how to use our lobby button system, to alert me that you are here for future appointments. 

If you have any questions, concerns, or confusion on directions please call or text me 360-466-8268.

 I will provide you with a link to my online HIPAA-compliant video platform, through Simple Practice. At the time of your online appointment, you will click the link and be directed into my virtual waiting room, there is nothing more you need to do on your end. I will then be able to start the teletherapy session with you.

You will be e-mailed a link to a client portal. I use Simple Practice, which is a HIPAA-compliant software that allows clients to electronically complete all paperwork.

Once you create a log-in, you will be able to complete all intake forms I have sent you. This should take about 15 minutes.

PLEASE COMPLETE ALL FORMS PRIOR TO YOUR FIRST SESSION. 

If you prefer hard copies, please let me know ahead of time, and I will be happy to accommodate.
